Please provide detailed information about your team's history in the Fun Cup championship (this will provide a basis for your review on the team line up section of the website funcup.co.uk ). You may also add any additional information that you would like to be included about your team on the website.

I wish to apply for registration for the 2012 Fun Cup Championship and agree for Fun Cup UK to debit my credit/debit card for the registration fee of £350 plus VAT. A member of staff will contact the Team Manager for card details. Completion of this form will be taken as the teams agreement to comply with all rules and regulations of the Fun Cup championship. Also, the credit/debit card details that you provide over the phone, will be used to take payment for all JPR Group invoices and unless otherwise informed, these card details will also be used for all entry fees during the 2012 season.
